  • Assisting with Self-Administered Medication certificate
  • Alzheimer's and Related Disorders certificate.

  • Job Description

    POSITION SUMMARY:

    Provides personal care services under the direction of the Registered Nurse. The Home Health Aide is assigned to specific clients by the Registered Nurse or other appropriate professional and performs services for clients as necessary to maintain their personal comfort.

    Reports to: Client Services Director/Nursing Supervisor

    QUALIFICATIONS:

    • Successful completion of a formal certification training program/written skills test and competency evaluation.
    • Be at least eighteen (18) years of age.
    • Minimum of six (6) months work experience in a supervised setting, preferably a health care facility.
    • Demonstrated ability to read, write and follow a written Plan of Care.
    • Good interpersonal skills.
    • Reliable transportation.

    ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY:

    • Wound Care
    • Apply Compression wraps
    • Performs personal care activities, including but not limited to:
    • Bathing
    • Shampooing
    • Skin care/nail care
    • Oral hygiene
    • Shaving
    • Dressing
    • Performs household services essential to health care at home, including but not limited to:
    • Meal preparation/feeding
    • Laundry
    • Light housekeeping
    • Reports any observed or reported changes in the client's condition/needs to the Registered Nurse.
    • Documents cares provided and completes the forms required for the client's record. Completes the appropriate records to document cares given and pertinent observations.
    • Promotes personal safety and a safe environment for clients by observing infection control practices, following agency guidelines and reporting unsafe situations to the supervisor/Case Manager.
    • Demonstrates safe practice in the use of equipment. Does not use equipment until orientation has been provided. Notifies supervisor of educational needs.
    • Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings and team conferences, as requested.
    • Maintains confidentiality in all aspects of the job.
    • Participates in required in-service programs to meet compliance requirements. Identifies learning needs to agency supervisor.
    • Performs other related duties and responsibilities as deemed necessary.

Homemaker/Companion

POSITION

Caregivers perform tasks that contribute to clients' comfort and safety in the home. Staff provides emotional support, socialization, quiet time, help with errands, cleaning and organizing

  • Demonstrate ability to read, write, follow a written Plan of Care and document tasks performed.
  • Must pass background check.
  • Complete tuberculosis screening
  • Be self-directed and work with minimal supervision.
  • Demonstrate dependability, patience and the ability to follow instructions.
  • Possess good interpersonal communication skills.

Job Description

 Trained Medication Assistant

Assist licensed nurse (RN) with administering prescribed drugs while providing safe and ethical care for clients in their homes .

*Must have certification to administer meds*

Essential Home Health Functions

  • Dispense oral, inhalation and topical medications to patients under direct supervision of a licensed nurse unless otherwise allowed by state law.
  • Observe patients for drug reactions and report any problems to the nurses.
  • Performing tasks associated with medication administration, including patient vital signs, calculate medication dosages and stock the medication cart.
  • Recording medication administration according to agency procedure
  • Assists the nurse in providing care to clients, which includes observation and reporting of client needs
  • Recognizes and performs tasks according to level of education and training
  • Accepts responsibility and accountability for own performance according to state laws and regulations.
  • Performs tasks in an ethical, legal and caring manner
  • Performs tasks safely and effectively to assure client comfort and welfare
  • Respects client rights • Protects confidential client information unless otherwise required to promote client safety and welfare
  • Follows federal, state, and agency regulations to protect own health and that of others

RESPITE CARE

Our Respite Care Service offer families the opportunity to take a break from their caregiving responsibilities, leaving their family member or loved one in the hands of a trusted professional caregiver. Our Respite care services are tailored to suit the needs of all our clients no matter what their needs may be.

Alternative Care (AC)

For older Minnesotans who require the level of care provided in a nursing facility and who are not yet eligible for Medical Assistance.

Community Access for Disability Inclusion (CADI) Waiver

For people with disabilities who require the level of care provided in a nursing facility.

Elderly Waiver (EW)

For older Minnesotans who require the level of care provided in a nursing facility.
